To open an account, a Resolution and Password Designation form(s) must be completed and submitted with original signatures to the address below. Upon receipt of the completed documents, each authorized user will received an email with user validation and initial log in instructions.

Resolution
The Resolution needs to be adopted by your governing authority. The Resolution authorizes the entity to establish an account with the MIP, and authorizes designated officers or employees of the depositor to invest or withdraw funds. The resolution must be attested. Password Designation(s)
Each representative of the depositor authorized to make deposits and withdrawals from the depositor's account must complete one of these forms designating their own unique verbal password.
